How to Submit A Claim:

Employees should visit Supplemental Health Plan Claims | Cigna Healthcare.
The claim forms and checklists will appear mid-page. Members can print these forms, complete in entirety and email them to SuppHealthClaims@Cignahealthcare.com with accompanying medical records for optimum processing time.
